• your name,

• your affiliation,

• your gender (optional),

• whether you are a masters' student, PhD student, postdoc, or other,

• the name of your adviser,

• a few lines about your motivation to participate in this conference,

• whether you need financial support,

• whether you need an official invitation (e.g. for your visa application),

• whether you want to participate in the guided city tour.
